Sexual Assault Incident Report Form

The form provided here is for those who wish to report a sexual assault anonymously. Please read through the defined defenses below and fill out the form completely.  Your report will be sent anonymously to the HCC Police Department. If you wish to make a report through campus police, you may dial 911 from any on campus phone and you will be connected with an HCC police officer.

Jeanne Clery Act Defined Forcible Sex Offenses

Report any sexual act directed against another person, forcibly and/or against that person's will; or not forcibly or against the person's will where the victim is incapable of giving consent. Do NOT differentiate between attempted and completed sex offenses.  All offenses must have occurred in a known campus location - incidents that occur in unreported/unknown locations will not be counted per Clery.

Four categories:

    • Forcible Rape: carnal knowledge of a person;
    • Forcible Sodomy: oral or anal intercourse;
    • Sexual Assault with an Object: use of an object or instrument (anything other than offender's genitalia - including finger, bottle, stick, etc) to unlawfully penetrate, however slightly, the genital or anal opening of the body of another;
    • Forcible Fondling: touching of the private body parts of another for the purpose of sexual gratification (includes "indecent liberties" and "child molesting") 

    Jeanne Clery Act Defined Non-Forcible Sex Offenses

    Report incidents involving unlawful, non-forcible sexual intercourse.

    Two categories:

      • Incest: non-forcible sexual intercourse between persons who are related to each other within the degrees wherein marriage is prohibited by law
      • Statutory Rape: non-forcible sexual intercourse with a person who is under the statutory age of consent - if force was used or threatened, or the victim is incapable of giving consent, the offense should be counted as forcible rape.

      Do NOT count sexual harassment, voyeurism or indecent exposure or anything else not specifically listed above.
