REGISTER WITH OSDDS

To receive accommodations and services, you must register with the Office for Students with Disabilities & Deaf Services (OSDDS). Stop by Donahue 147 OR contact us by calling 413.552.2417 to schedule and attend an information session. Please plan to bring with you to the information session:

  • Completed Student Information Form
  • A copy of documentation of your disability for OSDDS to keep on file. Your documentation should be relevant to your current status and include a specific diagnosis of a disability and specific recommendations for educational accommodations. Also see Disability Documentation Guidelines.
REQUEST YOUR SERVICES

Ready to request your approved services? This is where you officially request a classroom notetaker or an accessible version of your textbooks, if your OSDDS Specialist or another program on campus has approved you for these services. The following accommodations require the submission of an online request each and every semester:

  • Accessible Text
  • Notetaking Services
  • Accessible Furniture
accessible TEXT REQUESTS

To complete your request for accessible texts, you will need the following information:

  • Title
  • Author/Editor
  • Edition
  • Publisher
  • ISBN
  • Course
  • Instructor

The first five of these items should be on the first few pages of the book. You must fill out a separate request for each book that you need, and allow four weeks for your order to be completed. You must purchase your books in order to be provided with a PDF file version. Access a list of materials needed for each of your courses by going to the College Store, click on "Buy Books."

You will be redirected to the MBSdirect website:

  • Click "Let's Get Started"
  • Select the term (Ex. "Spring 2017"), program (Ex. Accounting "ACC"), course ID (select from dropdown list); repeat this for each course
  • Click "View Your Materials"

A list of all the information that you need for each of your courses will appear. If you have already purchased your books, go ahead and complete the form(s) by clicking on the link below. **Please note: You MUST purchase your books BEFORE submitting an alternative book request**

REQUEST NOTETAKING SERVICES

To submit requests for notetaking services you will need the following information:

  • Course (Ex. "ACC100.01")
  • Time (Ex. "8:30AM-9:20AM")
  • Day (Ex. "MWF")
  • Building (Ex. "DON")
  • Room (Ex. "147")
  • Instructor (Ex. "Smith")

This information is available on your schedule. Only include courses in which you need a notetaker.

Accessible Furniture Requests

To complete your request for accessible furniture, you will need the following information:

  • Student name
  • Email
  • Student ID
  • Your assigned OSDDS Specialist
  • Semester
  • Type of furniture
  • Building and room #
  • Course number & section
  • Day(s) of the course
  • Time(s) of the course 

This information is available on your schedule. You must fill out a separate request for each course so that OSDDS can be sure the furniture is available to you in all of your classrooms.
